Forms cannot be submitted online.Evidence submitted to the Forensic Science Division laboratory will be examined and analyzed in accordance with established
laboratory sampling plans and capabilities, utilizing validated methods and instrumentation. Method deviations will be utilized as necessary. As needed, laboratory personnel will contact the submitting agency to clarify requests or obtain additional information to assist in analysis.

DNA Consumption Template A letter or email from the County Attorney authorizing the possible consumption of samples collected from items submitted for DNA analysis will improve the turnaround time for your results. Items considered to be “touch DNA”, and also hair samples, all require this authorization prior to DNA analysis. A completed, signed (by the County Attorney) copy of this letter may be included with this submission form when applicable. 09/15 PDF
